What Is an Adult-Gerontology Primary Care Nurse Practitioner?

An Adult-Gerontology Primary Care Nurse Practitioner is an advanced practice registered nurse trained to care for adolescents or young adults through older adults, depending on program and certification language. AGPCNPs focus on primary care — meaning prevention, wellness, and ongoing management of long-term conditions rather than acute hospital care. You'll see patients across decades of life, building relationships that often span years, and you'll help them navigate everything from a first blood pressure diagnosis to complex aging-related health changes.

Day to day, AGPCNPs run wellness visits, manage chronic conditions like diabetes and hypertension, review medications for safety and interactions, order and interpret labs, perform screenings, prescribe within state scope, and educate patients and families. Depending on your state's APRN regulations, you may practice independently or in collaboration with a physician. Many AGPCNPs work closely with social workers, pharmacists, and specialists to coordinate care for older adults with multiple conditions.
